 PROFESSIONAL FEES

• The standard fee for the initial intake is $175.00 and each subsequent session is $150.00. However, family or couples therapy is $175 per session. Phone calls that last more than 10 minutes will be charged the same rate as a 45-minute session. Please note insurance providers do not cover phone conversations so this cost will be billed to you directly. You are responsible for paying at the time of your session unless prior arrangements have been made. Payment must be made by cash, PayPal, or credit card. If you refuse to pay your debt, I reserve the right to use an attorney, small claim court or collection agency to secure payment all additional fees that are used to collect a debt will also be the responsibility of the client.

Consultations will be assessed at a rate of $25.00 per 15 minutes. The fee will be due upon scheduling, and this will secure the appointment. In the event that an appointment is made, the fee will be credited towards any future services rendered.

Telephone calls longer than 10 minutes will cost $25.00. My fees for telephone calls

is $150.00 per hour; a 30-minute call will be $75.00.

APPOINTMENTS

Appointments will ordinarily be 50-55 minutes (this includes teletherapy) in duration, once per week at a time we agree on, although some sessions may be frequent as needed. The time scheduled for your appointment is assigned to you and you alone. If you need to cancel or reschedule a session, I ask that you provide me with 24-hour notice by calling my office and speaking to me or staff. If you miss a session without cancellation, or cancel with less than 24-hour notice, my policy is to collect the amount of $150.00 unless we both agree that you were unable to attend due to circumstances beyond your control. It is important to note that insurance companies do not provide reimbursement for cancelled sessions. The late cancellation fee or no-show charge is due immediately and if I must bill you there will be a $5.00 dollar charge. Thus, you will be responsible for the portion of the fee as described in this agreement. If it is possible, I will try to find another time to reschedule the appointment within that week. In addition, you are responsible for coming to your session on time. If you are late, your appointment will still need to end on time, and you are still responsible for your full copayment. Your copayment is due at the time of your appointment and if I must bill you there will be a $10.00 dollar fee per session. Also, please note it is your responsibility to know your copayment prior to coming in for your session. If for some reason Journey Two Serenity must wait for your insurance to be processed before Journey Two Serenity can receive your co-pay paid there will be a $10 fee for every co-pay that was not paid at the time of the service.

MEDICAL RECODES

• Cost and fees for records request range between $65-$150.00 for documents, varying by document type (records request is $0.31 per page, plus postage). A charge of up to $25.88 may be collected for administrative costs. In addition;

• A fee, not to exceed $9.70, for certifying the medical records may also be charged. In addition;

• The cost of postage may also be charged. In addition.

• Fees for copying documents may be:

• $0.97 per page for the first 20 pages

• $0.83 per page for pages 21 through 100

• $0.66 for each page copied in excess of 100 pages.

• For medical records that are not in paper form, the provider shall be entitled to recover the full reasonable cost of reproduction.

WRITING A TREATMENT SUMMARY

• Cost is $ 100.00

WRITING A LETTER | FMLA or Any Forms

• Cost ranges between $95 and $150 dollars. FMLA paperwork generally requires a minimum of 30 minutes to complete, due to the need for supporting clinical documentation. Short-term disability often takes longer to complete and may require additional assessments beyond my regular intake evaluation. The time required to make copies or prepare and send faxes, and any other administrative business (e.g. preparing releases of information or requests for records; phone calls to lawyers or other non-clinical calls) not directly related to the provision of clinical services, will also be assessed based on a rate of $150.00, with a minimum fee of $25.00. Please note I only write letters for clients that have been in treatment with me for at least two months consistently and is following their treatment plan. This also includes completing documentation for your primary insurance provider for yourself and/or your family member.

Expedited Documentation Request

Expedited request for a response or documentation within 1 business day is available for additional fee of $300.00.

Expedited request for a response or documentation within 3 business days is available for additional fee of $150.00.

Psychological Evaluations/Court Assessment

  • Initial Evaluation/Court Assessment Code 96101 $260.00
  • Psychological Evaluation/Court Assessment updated Code 96130 $160.00 per hour.
  • Psychological Evaluation-Court Assessment updated code +96131 $260.00 per hour

ATTENDANCE IN COURT

I can only testify to the facts of the case and to my professional opinion. Here are the following fees for me attending court.

1. Preparation time (including submission of records): $220/hr.

2. Phone calls: $220/hr.

3. Depositions: $250/hour

4. Time required in giving testimony: $250/hour

5. Mileage: $0.40/mile

6. Time away from office due to depositions or testimony: $220/hour

7. All attorney fees and costs incurred by the therapist as a result of the legal action.

8. Filing a document with the court: $150

9. The minimum charge for a court appearance: $1500

A retainer of $1500 is due in advance. If a subpoena or notice to meet attorney(s) is received without a minimum of 48-hour notice, there will be an additional $250 “express” charge. Also, if the case is reset with less than 72 business hours’ notice, then the client will be charged $500 (in addition to the retainer of $1500).
